ABSTRACT

Objective:To investigate the effect of different angles of atomizer on the delivery rates and total delivery quantities of Tanreqing inhalation solution, so as to provide reference for the clinical use of this preparation. Method:Taking baicalin, ursodeoxycholic acid, caffeic acid as indexes, PARI Boy SX compression atomization inhaler (equipped with red core atomizing cup) and BRS2000 respiratory simulator were used, the effects of different angles of the atomizer (upper 15 degree, lower 15 degree, upper 30 degree, lower 30 degree, partial 15 degree, partial 30 degree, vertical) on the delivery rates and total delivery quantities of Tanreqing inhalation solution were investigated. The respiratory patterns of adults, children, infants and young children were selected to determine the delivery rates and total delivery quantities of three components in Tanreqing inhalation solution. Result:In the same atomization time, the delivery rates and the total delivery quantities of caffeic acid and ursodeoxycholic acid in Tanreqing inhalation solution were not significantly affected by the atomizer from different angles, but significantly affected baicalin. At the vertical angle, the delivery rate and total delivery quantity of baicalin were higher than the other angles. Under different respiratory modes, there were significant differences in the delivery rates and total delivery quantities of these three components in the inhalation solution. Compared with other respiratory modes, the delivery rates and total delivery quantities of baicalin, ursodeoxycholic acid and caffeic acid were the highest in the adult respiratory mode, with delivery rates of (555.5±16.61), (226.3±6.54), (26.1±0.32) μg·min-1 and total delivery quantities of (4 001.1±82.97), (1 754.9±63.73), (167.6±1.42) μg, respectively. Conclusion:The use angle of atomizer has a certain effect on the delivery rate and total delivery quantity of Tanreqing inhalation solution, so it is suggested that the vertical angle should be kept as far as possible in clinical use. Under the four respiratory patterns, the delivery rate and total delivery quantity of Tanreqing inhalation solution are different, suggesting that the atomization dose or atomization time should be adjusted according to the respiratory characteristics of the patients to ensure the safety and effectiveness of clinical medication.

ABSTRACT

Abstract: This work proposes a method to characterize the respiratory pattern of patients with chronic heart failure (CHF) to determine non-periodic breathing (nPB), periodic breathing (PB) and Cheyne-Stokes respiration (CSR) through non-linear, symbolic analysis of biological signals. A total of 43 patients were examined for their cardiorespiratory profiles, their ECG and respiratory pattern signals were processed, analyzed and studied for parameters that could be of potential use in clinical decision making, specifically in patient classification. Patients in the study were characterized through their cardiorespiratory signals, applying joint symbolic dynamics (JSD) analysis to cardiac beat and respiratory interval durations. The most statistically significant parameters across all groups were identified through a Kruskal-Wallis two tailed test (α = 0.05) and a linear discriminant analysis (LDA) classification method based on such parameters was developed. The best result achieved with this classification method uses 10 features to discriminate patients with a 97.67% Accuracy (Acc). The best features to discriminate among groups are related to cardiorespiratory interaction rather than just respiration patterns alone. Results further support the idea that abnormal breathing patterns derive from physiological abnormalities in chronic heart failure.


Resumen: El trabajo propone un método para identificar el patrón respiratorio de pacientes con insuficiencia cardiaca crónica (CHF) con la finalidad de determinar respiración no periódica (NPB), respiración periódica (PB) y respiración de Cheyne-Stokes (CSR) a través de análisis simbólico no lineal de señales biológicas. Se examinaron los perfiles cardiorrespiratorios de 43 pacientes; sus señales de ECG y patrón respiratorio fueron procesados, analizados y estudiados en busca de parámetros que pudieran ser de utilidad, específicamente en la clasificación de pacientes. Estos pacientes se caracterizaron por medio de sus señales cardiorrespiratorias, aplicando un análisis de dinámica simbólica de conjuntos al ritmo cardíaco y a la duración de los intervalos respiratorios. Los parámetros de mayor significancia estadística entre todos los grupos se identificaron a través de una prueba Kruskal-Wallis de dos colas ( α = 0,05) y mediante un método de clasificación por análisis discriminante lineal (LDA). El mejor resultado conseguido con este método utiliza 10 características para discriminar a los pacientes con una precisión de 97,67% (Acc). Las características para discriminar entre grupos estuvieron relacionados con la interacción cardiorrespiratoria más que con solo los patrones de respiración, respaldando así la idea de que los patrones de respiración anormales derivan de anomalías fisiológicas presentes en la insuficiencia cardíaca crónica.

ABSTRACT

El Proceso de Atención de Enfermería (PAE) que se presenta tiene como objetivo: Identificarlas necesidades alteradas, corregir, mantener y rehabilitar al paciente por medio de la aplicación de conocimientos y procedimientos, con la finalidad de que este recobre en el menor tiempo posible su salud, en este caso una lactante con neumonía. Hoy en día, la neumonía es la causa principal de muerte de niños menores de 5 años en todo el mundo, su principal afectación son los alvéolos pulmonares, lo que compromete el patrón respiratorio. Esta enfermedad si no es tratada oportunamente puede tener complicaciones como: fallo respiratorio agudo, absceso pulmonar, insuficiencia cardiaca congestiva. Es entonces cuando el personal de Enfermería juega un papel importante en la ejecución de intervenciones, que ayuden a mantener la permeabilidad y la estabilidad de la vía aérea. Se identificaron las necesidades prioritarias con base en los 11 patrones funcionales de Marjory Gordon; se diseñó un plan de cuidados individualizado encaminado al mantenimiento del patrón respiratorio, para este se utilizaron las taxonomías: NANDA, NIC y NOC. Los resultados obtenidos fueron satisfactorios, se logró disminución de la dificultad respiratoria, mejoría en la permeabilidad de la vía aérea, mantenimiento de la frecuencia cardiaca y prolongación de periodos de sueño; sin embargo debido a que la paciente presentaba una desnutrición severa con evolución de 6 meses, no pudo modificarse ese patrón y aunado a la gravedad del padecimiento falleció.


The Nursing Attention Process (NAP) aims to identify altered needs, and correct, maintain and rehabilitate the patient through the use of knowledge and procedures, so that he/she can be healthy as soon as possible. In this study, the patient was an infant suffering from pneumonia. Currently, pneumonia is the world's principal cause of death in children younger than 5 years old. Indeed, consequences of not treating this illness on time include acute respiratory arrest, lung abscesses, and congestive cardiac insufficiency. Here, is when the nursing staff has a crucial role at maintaining the permeability and stability of the respiratory airways of these patients. Priority needs were identified through the 11 functional pattern of Marjory Gordon. An individualized care plan was designed and aimed to the maintenance of the child's respiratory patterns. NANDA, NIC and NOC taxonomies were used. Satisfactory results included a decrease in the respiratory difficulty, an improvement in the airway permeability, the maintenance of the cardiac rate, and the improvement of sleep time. However, due to a 6 month previous malnutrition, and the complications of this illness, unfortunately this patient died.

ABSTRACT

A finalidade deste artigo é avaliar o efeito da expansão rápida da maxila (ERM) no padrão respiratório. Por intermédio de um caso clínico, será relatado como indivíduos com atresia da maxila e problemas respiratórios podem se beneficiar com a expansão rápida da maxila. Outro aspecto que deve-se salientar é como profissionais da área da saúde, principalmente ortodontistas e otorrinolaringologistas, têm à sua disposição exames complementares para o diagnóstico do paciente com "Respiração Bucal".


The aim of the present investigation is to evaluate the effect of rapid maxillary expansion (RME) on the respiratory pattern. A clinical case is presented to describe how patients with atresic maxilla and respiratory problems can benefit from rapid maxillary expansion. The article highlights that the health professional, mainly the Orthodontist and the Otorhinolaryngologist, may use complementary exams to diagnose a mouth breather patient.
